The JetBlue Challenge Champion Form must be completed on-line at www.jetbluechallenge.com within ten (10) days following your tournament. Your failure to do so will result in your JBC Champion's non-participation in the Round III portion of the JetBlue Challenge. You may also fax this form to Colana Sports Group, Inc. at (631) 584 - 9544. Colana Sports Group, Inc. reserves the right to submit all prize winners and officials (witnesses) to a polygraph test for verification purposes. Thank you.
